In cases where the woman is under 35 years of age, it is recommended to consult with a specialist after a year in which she has been looking for a pregnancy without getting it. If you are over 35 years old, it is advisable to perform a basic fertility check, if you have not achieved it in six months.

It is also advisable after a period of six months in cases, regardless of age, in which there are previous risk factors, such as endometriosis, polycystic ovary syndrome, previous gynaecological surgery, spontaneous abortion…

In the case of a male member of the couple, consultation is advisable if they have not achieved a pregnancy in the period of one year of normal sexual relations with their partner, or in six months in cases of prior surgery on the genital tract, Mumps in childhood, difficulties in erection and also in cases of concomitant pathology such as obesity, hypertension or diabetes.

In women, the study includes an ultrasound evaluation to evaluate the ovarian reserve (the number of antral follicles that the ovaries have) and the uterus, ruling out uterine malformation or pathologies such as fibroids, adenomyosis or polyps. Normally a blood test for antimullerian hormone is also requested to complete the information about the ovarian reserve.

One of the most important parameters in the evaluation of ovarian reserve is age, since there is an inversely proportional relationship between the age of the patient and the number and quality of her ovules.

Sometimes, given certain background in the patient’s history or after a failure of the assisted reproduction technique, it is necessary to expand the studies, and maybe requiring:

  • Hysteroscopy
  • High resolution ultrasound
  • Thrombophilias study
  • Immune study
  • Study of endometrial receptivity
  • Endometrial micro biota study
  • Study of chronic endometritis

Having a normal seminogram in terms of quantity, morphology and mobility does not guarantee that the semen is fertile, sometimes it is necessary to deepen the male factor with tests such as:

  • Sperm FISH
  • Halo sperm for fragmentation study
  • COMET for study of single and double chain fragmentation

Alterations in the embryo are the cause of more than 50% of cases of abortion and implantation failure. The transfer of chromosomally normal embryos greatly increases the pregnancy rate regardless of age, and dramatically reduces the possibility of having an abortion later.

When evaluating patients presenting with these conditions, it is important to try to obtain the maximum information from both ovum and sperm, since both contribute to the chromosomal load of the embryo.

However, there are other causes that can directly affect the mother and cause these abortions or implantation failures. The first studies to be carried out would be an in-depth evaluation of the uterus and endometrium and specific analyses to rule out alterations in blood clotting. In more complex cases it may be necessary to resort to immunological studies or more advanced studies of the endometrium.

At FIV Marbella we offer a comprehensive study of these complex cases, reaching in the vast majority of cases an appropriate diagnosis and treatment.

Anxious, frustrated or especially expectant. These adjectives might reflect how most couples who attend an assisted reproduction clinic are. The difficulty in conceiving a child is an unexpected situation that can lead to many reactions, each of which will involve different needs. That is why, from the outset, a psychological evaluation is essential that allows us to be aware of the emotional situation from which we start, the implications that the assimilation of the problem, the treatment itself or even the results may entail.

Through different evaluation techniques, we can obtain a clear vision of both the initial emotional state and the personal and social resources of both members of the couple, aspects that will be essential in the programming of the intervention and / or appropriate psychological support for each case.

Likewise, it is a first contact where to create an environment of acceptance, security and trust, since a very important point of treatment is to have a space where you can express emotions, concerns, develop coping strategies and solve all kinds of doubts That is, to have constant support and advice throughout the entire process.

The first visit is made with a gynaecologist specialized in infertility, who will be responsible for your treatment.

First of all, your medical history will be evaluated, paying special attention to everything that may be important in the diagnosis of your infertility problem. In case of having carried out any previous study or treatment, all the details will be observed to have the maximum possible information.

If the gynaecologist considers it useful, an ultrasound will be performed free of charge during this visit to evaluate the uterus and ovarian reserve.

With all the information, the gynaecologist will determine if additional tests are necessary and will guide you towards the most appropriate fertility treatment in your case.

At the end of the consultation, the Patient Care Department will give you all the information regarding the budgets of the indicated techniques and treatment, as well as the tests that the doctor requests. Most of these tests can be done on this same day.
